Forms and Further Information

M.S. Students:

• Departmental M.S. Check List (pdf)
• Transfer of Credit (pdf) (up to 9 hrs of graduate wrok may be transferred in from another school or from post-bac status) (please type and submit to the DGS)
• Departmental Thesis Proposal Form (to accompany your thesis proposal; proposals are due Feb 15 for students entering in Fall)
• Application for Degree Card (pdf) (please type and submit to DGS for signature; must be submitted within the first 30 days of the semester in which you plan to graduate, or 15 days in the case of the summer sessions; cards good for one year).
• Final Exam Recommendation (pdf) (please type and submit to the DGS 3 weeks prior to your exam; confirm the date, time, and location of the exam before submission).
• Departmental Thesis Defense Approval Form (please type, obtain signatures, and submit to DGS 3 weeks prior to your exam)

Ph.D. Students:

• Departmental Thesis Proposal Form (to accompany your thesis proposal; proposals are due before the Qualifying Exam)
• Doctoral Advisory Committee (pdf) (please type, obtain signatures, and submit to DGS; must be established prior to scheduling the Qualifying Exam)
• Recommendation for Qualifying Exam (pdf) (please type, obtain signatures, confirm room and time, and submit to DGS; the time for the exam must be scheduled AND approved by the Grad School within the first 6 weeks of the semester; the actual exam may be taken at any time during that semester; candidates need to be signed up for GLY 767 (2 hrs) that semester ans each subsequent semester until graduation).
